La Organización Mundial de la Salud (OMS) define a la infertilidad como la incapacidad de lograr un embarazo clínico después de 12 meses o más de relaciones sexuales sin protección. Su prevalencia global a lo largo de la vida es del 17,5% (~1 de cada 6 personas de la población adulta), lo que muestra la necesidad urgente de aumentar el acceso a servicios de diagnóstico y tratamiento de infertilidad asequibles y de alta calidad. Hasta la mediana edad (de 40 años a 65 años), la incidencia de infertilidad es casi igual en mujeres y hombres, 35% y 30%, respectivamente; en el 20% de los casos intervienen factores combinados, y en el 15% se desconoce su causa. Específicamente, la infertilidad masculina es una patología clínica compleja, que afecta a la persona, su pareja y la descendencia. El trabajo de Kimmins y colaboradores surge de una propuesta de investigación en infertilidad masculina del MRHI, Iniciativa en Salud Reproductiva Masculina (del inglés Male Reproductive Health Initiative(1), grupo de trabajo de la ESHRE, del que soy miembro junto con colegas del área de salud reproductiva masculina de todo el mundo, en un esfuerzo por proporcionar claridad al área científico y médica, así como a los gobiernos y al público sobre el estado actual de la investigación y abordajes de la medicina en Andrología, y dónde los recursos futuros podrán utilizarse de forma más eficiente. Según esta Recomendación de Expertos publicada en la revista Nature Reviews Urology, en la actualidad la mayoría de los hombres con infertilidad no conocen la etiología de su enfermedad. Además, existe una falta de reconocimiento de la carga de la infertilidad masculina y su potencial como biomarcador de enfermedades sistémicas. La ausencia de tal conocimiento da como resultado que los pacientes generalmente sean tratados como un grupo uniforme, para quien la estrategia es evitar la causalidad utilizando técnicas de Reproducción Médicamente Asistida (RMA). De esta forma, se pierden oportunidades para prevenir la comorbilidad y la carga del tratamiento se traslada a la mujer.
